**Job Summary:**
We are seeking a dedicated and professional Physical Therapist to provide coordinated care to patients/clients of all age groups in the Westminster, Arvada, Thornton, Wheat Ridge, Northglenn, CO and surrounding areas.
**Key Responsibilities:**
* Develop and implement individualized physical therapy treatment plans.
* Evaluate and document patient/client progress, adjusting treatments as necessary.
* Coordinate care with other healthcare professionals to ensure comprehensive treatment.
* Educate patients/clients and their families on treatment plans, home exercises, and preventative care.
* Maintain accurate and timely patient records in compliance with company and regulatory standards.
* Utilize durable medical equipment as needed and provide training to patients/clients.
* Ensure patient safety and comfort during therapy sessions.
* Participate in continuous professional development and stay updated with the latest therapy practices.
**Requirements:**
* Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
* Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, or meets one of the requirements listed in 42 CFR § h).
* At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
* Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.
* Reliable transportation, a current Driver's License, and valid and current auto liability insurance.
