About the Role

We are seeking a skilled Clinical Health Navigator to join our team in the Behavioral Health Care Management Department. As a key member of our interdisciplinary Care Management team, you will be responsible for developing and managing individualized intervention plans for members referred into our Behavioral Health (BH) Care Management program.

Key Responsibilities:

  • Develop and manage intervention plans to promote optimal outcomes in the most cost-effective and appropriate manner.
  • Work collaboratively with the Care Management team to facilitate quality outcomes across the continuum of care by educating members, providers, external agencies, and internal departments.
  • Participate in Quality Improvement efforts to continually evaluate the effectiveness of our Care Management Program and its goals and outcomes.

Requirements

To be successful in this role, you will need:

  • Knowledge of principles and practices of advocacy and case management, including assessment, diagnosis, care/treatment planning, discharge planning, and documentation.
  • Familiarity with clinical evidence-based practices, including developing a plan of care, motivational interviewing techniques, and use of the DSM 5-TR for diagnostic purposes.
  • Ability to use pro-active customer service skills in handling complex and demanding situations.
  • Strong critical thinking and problem-solving skills, as well as the ability to navigate computerized Electronic Health Records (EHRs).
