Key Responsibilities:
As an RN Hospice Case Coordinator, you will embody our Mission of Love by ensuring high-quality care for our hospice admissions in a compassionate, timely, and suitable manner, with a focus on managing patients' symptoms and comfort levels. You will also be encouraged to propose ideas for organizational improvements.
Your primary duties will include:
- Maintaining up-to-date knowledge of hospice care standards to ensure compliance with Medicare certification and state licensure requirements. Documenting assessments, certifications, and care plans in line with Medicare Conditions of Participation.
- Participating in interdisciplinary group meetings, mandatory staff gatherings, and training sessions.
- Communicating effectively with patients' families, primary care physicians, facility staff, and hospice medical directors regarding updates or changes in patient conditions.
- Monitoring the quality of end-of-life care by regularly assessing patients' needs, ensuring accurate documentation, and coordinating care with interdisciplinary team members. Providing education on end-of-life issues to patients, families, caregivers, and friends; addressing the physical, mental, social, and emotional needs of terminally ill patients and their loved ones.
- Regularly revising and updating care plans while assessing family and caregiver needs.
- Pronouncing death in the absence of a physician and ensuring all necessary notifications are made, along with completing required documentation.
- Administering and documenting professional nursing care to assigned hospice patients in accordance with nursing processes and administrative policies.
- Delivering patient care in a manner that prioritizes safety and minimizes the risk of medication and healthcare errors. Demonstrating knowledge of medications and their proper administration. Coordinating all patient services and assessing pain to implement appropriate pain management strategies. Offering support and palliative care.
Qualifications for the RN Hospice Case Coordinator position include:
- Current RN licensure from the Pennsylvania State Board of Nursing.
- CPR certification required prior to or upon employment.
- Previous experience in hospice care.
- Valid Pennsylvania driver's license and insurance for personal vehicle.
- Strong mentorship program available - training will be provided.
