Nurse Home Health Specialist

We are seeking a skilled and compassionate Nurse Home Health Specialist to join our team in New York. As a Travel RN, you will have the opportunity to make a meaningful impact on patients' lives while exploring new destinations.
Compensation:
$80.00 per hour (based on 12-hour shifts and 36 hours per week) with tax-free stipend amount to be determined
About the RoleThis position involves providing high-quality care to patients in their homes, conducting comprehensive assessments, and developing individualized long-term care service plans. You will work closely with interdisciplinary teams to ensure seamless care coordination and efficient service delivery.
Responsibilities- Initial Assessments: Conduct thorough pre-evaluations of patients in their homes, assessing their health status, strengths, care needs, and preferences
- Medical Documentation: Review consumer medical documentation and health referral forms as relevant to the case
- Evaluation Data Entry: Enter evaluation data into electronic evaluation forms and transmit as required
- Case Notes: Document concerns, conflicting information, and other issues that surface during the evaluation process
- Care Coordination: Emphasize continuity of care, reducing or eliminating fragmentation, duplication, and gaps in treatment plans
- Patient Education: Discuss healthcare options, supports needed, service vendor options, and waiver options with patients
- Communication: Initiate communication between the Call Center and eligible consumers for enrollment into managed care health plans
- Quality Review: Collect quality review data and required documentation to support outcome measurements and record case notes
- Community Resources: Maintain a working knowledge of community resources, payor requirements, and network services for the target population
- Performance Standards: Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template
Requirements:
- Current RN licensure in New York State
- Two years of experience in a HOME setting within the last fifteen years
- Recent experience conducting comprehensive assessments of patients
- Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment
- Knowledge of state and agency laws and regulations governing general professional nursing practices
