About the Role

CenterWell Home Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.

Key Responsibilities
  • Plan and administer prescribed physical therapy treatment and training for patients to attain the highest level of physical function.
  • Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
  • Develop and implement a conditioning and rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
  • Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
  • Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervise and evaluate their performance, and monitor the completion of documentation.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations, and ensure that interim orders are promptly and accurately documented and implemented.
  • Report patients' progress to physicians, Clinical Managers, staff, patients, and families, and submit evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
Requirements
  • Graduate from an accredited Physical Therapy Program approved by the APTA.
  • Minimum of one year of physical therapy experience, with Home Health experience preferred.
  • Current and unrestricted Physical Therapy licensure and CPR certification.
  • Strong organizational and communication skills, and a valid driver's license, auto insurance, and reliable transportation.
