Job Summary:

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at HexaQuEST Health, Inc. in a Skilled Nursing Facility (SNF) setting. As an LVN, you will play a vital role in providing high-quality patient care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities:

• Conduct individualized patient assessments, prioritizing data collection based on the patient's immediate condition or needs.
• Develop and implement individualized plans of care, collaborating with other healthcare professionals to ensure comprehensive care.
• Perform various nursing procedures, including administering medications, providing wound care, and conducting vital signs.
• Maintain accurate and timely documentation of patient care, including assessment findings, interventions, and progress notes.
• Respond to emergency situations, initiating resuscitation measures as needed and documenting the incident.
• Collaborate with the Charge Nurse and other healthcare professionals to ensure seamless care transitions and effective communication.

Requirements:

• Current LVN licensure in the state of California.
• BLS certification.
• Ability to work effectively in a fast-paced, dynamic environment.
• Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
• Ability to maintain confidentiality and handle sensitive information with discretion.
• Willingness to work beyond normal working hours and in other positions temporarily, as needed.
