Job Summary

Promote and restore patients' health by completing the nursing process, collaborating with physicians and interdisciplinary team members, providing physical and psychological support to patients, friends, and families, and delegating and evaluating care assigned to LPNs and CNAs to meet patient care needs.

Essential Functions
  • Identify Patient Care Requirements
    • Establish personal rapport with potential and actual patients and other persons in a position to understand care requirements.
  • Use the Nursing Process
    • Establish or contribute to a plan of care with the patient, including a teaching plan, within 24 hours of admission.
    • Establish or contribute to prioritization of goals appropriate to the patient's age and ability to learn.
    • Identify nursing care needs, write nursing interventions, and identify educational objectives appropriate to care standards.
    • Reassess and document the effectiveness of nursing actions and the patient's progress toward goals as the patient's condition changes.
    • Communicate the plan and patient progress to the interdisciplinary team.
    • Delegate the plan of care to other licensed and unlicensed associates within their scope of practice.
  • Establish a Compassionate Environment
    • Provide emotional, psychological, and spiritual support to patients, friends, and families.
  • Promote Patient's Independence
    • Establish patient care goals.
    • Educate patients, friends, and family to understand condition, medications, and self-care skills.
    • Answer questions by applying rehabilitation principles to patients served.
    • Demonstrate competence in providing age-specific care to pediatric patients, adult patients, and geriatric patients.
  • Assure Quality of Care
    • Adhere to therapeutic standards.
    • Measure health outcomes against patient care goals and standards.
    • Make or recommend necessary adjustments.
    • Follow hospital and nursing division's philosophies and standards of care set by state board of nursing, State Nurse Practice Act, and other governing agency regulations.
    • Participate in performance improvement activities.
    • Direct and assess care provided to assigned patients by LPNs, Nursing Assistants, agency personnel, students, and other associates.
  • Resolve Patient Problems and Needs
    • Utilize multidisciplinary team strategies.
    • Utilize established communication methods to ensure resolution of patient problems and needs.
    • Communicate patient status to the interdisciplinary team.
  • Document Patient Care Services
    • Document delivery of services, changes in the patient's condition, and the patient's response to nursing interventions in the medical record according to policies and procedures.
  • Maintain Patient Confidence and Organizational Confidentiality
    • Keep information confidential.
  • Maintain a Safe and Clean Working Environment
    • Comply with procedures, rules, and regulations.
    • Call for assistance from healthcare support personnel.
    • Maintain a clean and safe unit environment.
    • Keep patient rooms, supply rooms, and hallways free from clutter.
  • Ensure Operation of Equipment
    • Follow manufacturer's instructions.
    • Troubleshoot malfunctions.
    • Enter work orders for equipment repairs.
    • Evaluate new equipment and techniques.
  • Demonstrate Financial Accountability
    • Check stock to determine inventory level.
    • Anticipate needed supplies.
    • Use equipment/supplies as needed to accomplish job results.
    • Minimize overtime by work prioritization and delegation.
    • Recognize and comply with RN's role in reimbursement for patient care services.
    • Identify and provide input to Nurse Managers on work methods that reduce the cost and/or time required to deliver patient care.
    • Maintain awareness of unit budget.
  • Maintain Professional and Technical Knowledge
    • Identify personal educational needs and opportunities.
    • Attend educational workshops.
    • Complete mandatory educational requirements.
    • Review professional publications.
    • Establish personal networks.
    • Participate in professional organizations.
    • Share new knowledge with teams.
    • Attend unit meetings.
    • Read unit memos.
    • Participate in committees, projects, and PI activities.
  • Maintain a Cooperative Relationship Among Healthcare Teams
    • Communicate information.

    • Communicate patient care or team issues to the nurse manager or supervisor.

    • Utilize the chain of command to present issues or concerns.
    • Demonstrate positive conflict resolution skills with all associates.
    • Treat all associates with dignity and respect.

    • Arrive on duty as scheduled, maintain attendance, punch in and out as per Good Shepherd policy, and maintain time sheet.
    • Demonstrate flexibility in response to change by offering to assist team members with patient care needs and unit activities that arise throughout the shift.

    • Attend meetings as requested by organizational leaders.

Qualifications

  • Education
    • Associate's Degree in Nursing or Nursing Diploma required.
    • Bachelor's Degree in Nursing preferred.
  • Work Experience
    • Prior experience in med-surg or rehabilitation experience preferred.
  • Licenses/Certifications
    • Registered Nurse license required.
    • Basic Life Support/CPR, as a healthcare provider, per the American Heart Association required.
    • Certification in Rehabilitation Nursing preferred (must be acquired with 3 years of employment).
