Job Title: Home Health Aide

At A Place at Home, we are seeking a compassionate and dedicated Home Health Aide to join our care team. As a Home Health Aide, you will play a vital role in providing high-quality care and support to our clients in the comfort of their own homes.

Key Responsibilities:
  • Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
  • Assist with meal planning and preparation, observing dietary restrictions when necessary.
  • Assist with medication reminders in compliance with established care plans.
  • Perform light housekeeping tasks such as dusting, vacuuming, and laundry to maintain a clean and comfortable living environment for our clients.
  • Assist with mobility and transfers, ensuring client safety at all times.
  • Provide companionship and foster a nurturing relationship with our clients.
  • Document and report changes in client's physical condition, behavior, or living conditions to the supervising nurse.
  • Follow HIPAA guidelines and maintain the privacy of all client PHI (Personal Health Information).
  • Participate in ongoing training and professional development opportunities.
Requirements:
  • Completion of state-approved Home Health Aide certification course.
  • Strong communication skills, both written and verbal.
  • Compassionate and empathetic with a strong desire to improve the lives of others.
  • Ability to work independently and maintain high levels of patience and understanding.
  • Physically able to perform the duties of the position, including lifting and physical movement as necessary.
  • Reliable transportation and valid driver's license.
Physical Demands:

The position involves moderate physical activity such as bending, lifting, and walking.
