**Key Responsibilities**
- Collaborate with healthcare providers, hospitals, and care facilities to ensure seamless transitions and quality home-based services.
- Work closely with discharge coordinators to coordinate home care services, empowering patients to make informed decisions about their care.
- Provide patients and families with education and support on home care regulations and services, promoting independence and self-advocacy.
- Develop and maintain strong relationships with patients, families, and healthcare teams to ensure comprehensive care and support.
