About the Role:

This is a field-based position that requires working with Medicaid members to ensure they receive the appropriate care. You'll act as a liaison between the member, their family, and healthcare providers to provide home and social assessments, education, and support. Your goal will be to help members manage their health and healthcare needs, while also addressing social determinants of health such as transportation, housing, and food access.

Responsibilities:
  • Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate care
  • Utilize company and community-based resources to establish a safe and effective case management plan for members
  • Collaborate with members, families, and healthcare providers to ensure coordinated care
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Keep members out of the hospital by supporting regular visits to their primary care physician
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team
Requirements:
  • High School Diploma / GED (or higher) OR 5+ years of community outreach experience
  • 2+ years of experience with knowledge of the resources available, culture, and values in the community
  • 1+ years of experience with electronic charting
  • Intermediate level of computer proficiency including MS Word, Excel, and Outlook
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Valid state of Michigan driver's license with state required insurance and access to reliable transportation
Preferred Qualifications:
  • Ability to work within a matrix environment where team members are engaged in the success of the team as indirect reports but have a direct reporting relationship to others
  • Community Health Worker (CHW) State course completion
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
  • Background in managing populations with complex medical and behavioral needs
  • Knowledge of Medicaid population
