About the Role
We are seeking a skilled Admissions Nurse to join our team at Bethesda Health. As a key member of our admissions team, you will be responsible for ensuring a seamless transition for patients and their families as they enter our care. Your expertise will be instrumental in assessing, planning, implementing, and evaluating patient plans of care, while providing exceptional customer service and communication with families, nursing home staff, social workers, and other team members.
Key Responsibilities
• Conduct thorough head-to-toe assessments to identify patient needs and develop individualized plans of care
• Coordinate communication with families, nursing home staff, and other team members to ensure a smooth admission process
• Complete initial plans of care, medication lists, and other necessary documentation in our electronic medical record system
• Verify physician orders and ensure accurate and timely implementation of patient care plans
• Collaborate with interdisciplinary teams to ensure comprehensive care and address patient needs
Requirements
• Graduate of an accredited school of nursing
• Current nursing license in good standing in the state of practice
• Minimum two years of varied work experience as a professional nurse, preferably in a post-acute setting
• Excellent communication and documentation skills, with ability to work effectively in a fast-paced environment
• Ability to lift, transfer, and/or move patients with weights up to 50 pounds, using proper lifting techniques and seeking assistance when necessary
