Job Summary
We are seeking a highly skilled and compassionate Physical Therapist to join our certified home care team. As a Physical Therapist at Charlestown, you will have the opportunity to work with a diverse range of residents, providing person-centered care and support to help them achieve their goals and maintain their independence.
Key Responsibilities
- Provide holistic physical therapy services to residents, including assessment, treatment, and education.
- Develop and implement individualized care plans that address the physical, emotional, and social needs of residents.
- Collaborate with the interdisciplinary clinical team to ensure seamless care coordination and communication.
- Conduct regular progress assessments and adjust treatment plans as needed to ensure optimal outcomes.
- Provide education and support to residents and their families on physical therapy techniques and strategies for maintaining independence.
Requirements
- Minimum of one year work experience within a home health setting.
- Degree and training must meet current Medicare and state requirements.
- Must possess a valid and current state Physical Therapy license.
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements.
- Therapeutic experience in older adults across multiple diagnostic groups is preferred.
