About the Role

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. This is a dynamic opportunity for a licensed nurse to provide exceptional care and services to our residents while promoting a safe and organized environment.

Job Description
  • Administer medications in accordance with physician orders and resident care plans.
  • Document medication administration accurately and efficiently.
  • Provide resident care that aligns with Sunrise Senior Living's mission to champion quality of life for all seniors.
Responsibilities & QualificationsResponsibilities:
  1. Deliver high-quality care to residents, prioritizing their physical, emotional, and social well-being.
  2. Collaborate with healthcare teams to develop and implement individualized care plans.
  3. Maintain accurate and detailed records of medication administration and resident progress.
Qualifications:
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential required.
  • In states where applicable, must maintain certifications.
  • Minimum 6 months of experience working with seniors in assisted living, home health, independent living, hospital, or long-term care environments.

Compensation
