Job Summary:

YAI is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will be responsible for providing high-quality patient care to individuals with intellectual and developmental disabilities. Your expertise in wound care, medication management, and G-Tube administration will be invaluable in ensuring the well-being of our members.

Key Responsibilities:
  • Provide wound care and manage medications for our members
  • Administer G-Tubes and report any inconsistencies in our members' condition
  • Document reports and observations, and interact with patients and families
  • Teach appropriate self-care techniques and consult with Case Managers/MDs in determining plan of care
  • Notify Case Managers, MDs, and/or families of changes in members' condition
  • Maintain reporting systems and report abuse or unsafe/illegal practices
  • Make recommendations to improve quality of life for our members

Requirements:
  • NYS Licensed Practical Nurse (LPN) License
  • Med/Surg experience
  • Experience working with intellectually and developmentally disabled populations

  • Flexible scheduling, including every 3rd Saturday
