- Conducting comprehensive physical therapy assessments in line with APTA standards and state regulations.
- Creating individualized Treatment Plans tailored to each patient's needs based on initial assessments and ongoing evaluations.
- Implementing Treatment Plans consistently while making necessary adjustments based on patient progress.
- Maintaining accurate documentation of patient progress in accordance with established medical and home health care protocols.
- Assessing patient mobility, strength, and functional capabilities, while setting realistic and attainable improvement goals.
- Communicating patient progress effectively to patients, caregivers, and other healthcare professionals involved in their care.
- Formulating post-treatment plans that include appropriate in-home care services.
- Enhancing professional knowledge through continuous education and professional development opportunities.
- Collaborating with other healthcare staff to ensure a clean and effective rehabilitation environment.
The ideal candidate will possess:
- A proven track record of aligning patient needs with effective treatment strategies.
- Extensive experience with various physical therapy techniques, including but not limited to massage therapy, physical agents application, traction initiation, and hydrotherapy.
- Experience in supervising and guiding the work of aides and assistants.
- Knowledge of best practices for equipment use and maintenance.
- Experience in recommending assistive devices and follow-up care programs.
- Familiarity with diverse patient demographics and a range of injuries.
- An understanding of Occupational Therapy principles and their application.
- Empathy and a compassionate approach to patient care, with the ability to inspire patients to achieve their rehabilitation goals.
We require candidates to have:
- A BS or Master's Degree in Physical Therapy from an accredited program.
- A valid Physical Therapy license for the state of practice.
- Current Basic Life Support (BLS) certification, including AED training.
Compensation Structure:
- $175 per SoC Visit
- $100 per RoC/Eval Visit
- $90 per Regular Visit
- $35 per hour for Administrative Tasks
