Key Responsibilities- Assessment and Treatment Planning
Conduct thorough assessments of patients' social and emotional needs, and develop personalized treatment plans to address their unique challenges.
Individual and Group TherapyProvide one-on-one and group therapy sessions to patients, focusing on issues such as PTSD, anger management, coping skills, and personal interactions.
Case ManagementCoordinate with patients, families, and healthcare providers to ensure seamless care and support throughout the treatment process.
Community Resource ReferralIdentify and connect patients with community resources and organizations that can provide additional support and services.
Documentation and ReportingMaintain accurate and detailed records of patient interactions, treatment plans, and progress.
Requirements- Education and Licensure
A Master's degree in Social Work from a program registered by the Department of Education is required, with specific training dealing with adult patients.
Experience and SkillsA minimum of 1-3 years' post-master direct counseling experience with adult patients is required. Proven mastery of advanced sociological, psychological, and mental health care-related interventions and issues is essential.
Certifications and CredentialsCertification in Basic Life Support and ACLS is required, and proficiency in written and spoken English is necessary.
Computer SkillsStrong computer skills, including EMR experience, are required.
