Job Summary

We are seeking a highly skilled Registered Dietitian to join our team at Autumn Woods Health Care. As a key member of our Nutrition Services department, you will be responsible for ensuring that our residents receive high-quality nutritional care and support.

Key Responsibilities
  • Maintain accurate and up-to-date clinical nutrition documentation for residents, in accordance with company policies and state and federal regulations.
  • Develop and implement individualized plans of care, in collaboration with the interdisciplinary team, to meet the unique needs of each resident.
  • Conduct regular meal rounds and interviews with residents, family members, and staff to ensure that residents are receiving the food and nutrition they need to maintain or improve their health.
  • Collaborate with the Nutrition Services Supervisor/Manager to develop and customize menus that meet the dietary needs and preferences of our residents.
  • Monitor food temperatures and quality, and take corrective action when necessary, to ensure that our residents receive safe and nutritious food.
  • Assist with the processing of diet orders and changes, and ensure that accurate tray cards are available for each resident receiving meals.
  • Participate in the Nursing Center's Performance Improvement Program, and contribute to the development of action plans to improve nutrition services.
  • Communicate effectively with all departments in the Nursing Center, and provide feedback and suggestions for improving company policies and procedures.
Requirements
  • Registered Dietitian with the Commission on Dietetic Registration of the American Dietetic Association.
  • License or certification as mandated by the state in which employed.
  • ServSafe Food Safety Certification preferred.
  • Experience in long-term care environment and geriatric nutrition preferred.
