Nurse Care Coordinator Libertyville

We are excited to offer a unique opportunity for a skilled Nurse Care Coordinator to join our team at Advocate Aurora Health. In this role, you will play a vital part in ensuring that patients receive the highest quality care in their homes, focusing on preventing hospital readmissions and promoting overall health and well-being.
This is a challenging yet rewarding role, requiring a strong background in nursing and excellent communication skills. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans, coordinating services and resources to support patients in their recovery.
This is a full-time position working 40 hours per week, with opportunities for overtime as needed. We offer a competitive salary of $90,000 per year, plus benefits including holiday pay, guaranteed hours, continuing education, and a 401(k) retirement plan.
