Job Summary

The Physical Therapist for the AgeWell PACE program is responsible for evaluating and reassessing participants to determine their mobility level and functional abilities. This role involves providing and recommending therapeutic exercise, gait training, and ambulation, as well as planning and developing group exercise programs for maintenance therapy.

Key Responsibilities
  • Conduct initial assessments and reassessments of participants' condition and functional status, communicating changes to team members.
  • Develop treatment programs, set goals, and provide treatment, recording and signing treatments in participants' medical records.
  • Coordinate 24-hour care delivery, provide training to family members, friends, and AgeWell PACE staff members, and supervise program aides and other staff implementing PT treatment programs.
  • Review assessments and discharge participants when appropriate, participate in case conferences, and attend staff meetings to contribute to participant care planning.
Requirements
  • Graduation from an accredited physical therapy program and California Physical Therapist License upon hire.
  • Minimum 2 years' experience as a PT, with at least 1 year working with a frail or elderly population.
  • Licensed Physical Therapist in CA, valid CA driver's license, and vehicle.
  • Demonstrated clinical competency with geriatric populations and ability to work independently.
Work Environment

The Physical Therapist will work in a busy and demanding clinic environment with moderate to loud noise levels, requiring the ability to sit, stand, and walk, as well as lift, position, push, and/or transfer participants.
