We are seeking a highly skilled and compassionate Case Manager to join our team at BayMark Health Services. As a Case Manager, you will play a vital role in enhancing the overall treatment process by assisting therapists to meet the treatment needs of clients.
Key Responsibilities:- Act as a liaison between therapists, referral sources, and family members to ensure seamless communication and coordination of care.
- Arrange family visits, coordinate transportation, and ensure clients' needs are met in a timely and efficient manner.
- Attend court hearings as necessary and provide prepared reports that accurately represent clients' responses to treatment.
- Facilitate individual and group counseling sessions as assigned and provide support to clients in achieving their treatment goals.
- Attend supervisory sessions with prepared agendas and participate in ongoing professional development to enhance skills and knowledge.
- Assist clients in achieving personal independence in managing daily needs and facilitate further development of daily living skills as identified by the client and/or parent or guardian.
- Experience working with adults with addiction is preferred.
- Must have a CDCA certification.
- Excellent communication and interpersonal skills, with the ability to interact in a pleasant and professional manner with colleagues, physicians, providers, patients, and patients' families.
- Strong organizational skills, with the ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records.
- Satisfactory drug screen and background checks.
