One Home Health LLC is looking for a Practical Nursing Specialist to become a vital part of our healthcare team. In this role, you will be tasked with providing exceptional care to our designated patients.
Key Qualifications:
- Completion of an accredited nursing program.
- Valid license to practice as a Licensed Practical Nurse in the relevant state(s).
- At least one (1) year of experience in a hospital or similar environment; experience in community or home health settings is a plus.
- Current CPR certification.
- Strong written and verbal communication abilities, along with excellent interpersonal skills.
- Familiarity with Electronic Medical Records (EMR) systems and proficiency in using tablets for documentation and communication.
- Ability to interact sensitively with clients and the community.
- Strong organizational and time management capabilities.
- Must possess a valid driver's license and have an insured vehicle that meets state and/or agency standards, with a clean driving record.
Core Responsibilities:
- Deliver professional nursing care as outlined in the nursing practice regulations and under the supervision of a Registered Nurse.
- Report any changes in patient status or condition to the RN according to agency protocols.
- Ensure personal safety and maintain a secure environment for both clients and colleagues.
- Adhere to agency policies, procedures, and professional standards in all job functions.
- Participate in the coordination of home care services, attend necessary meetings, and comply with reporting requirements.
- Exhibit basic understanding and proficiency in using Glucometer and INR devices.
- Demonstrate knowledge and skills in wound care management.
- Perform additional duties as assigned.
