Job Title: Licensed Practical Nurse, LPN PRN

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a PRN (as needed) nurse. As a LPN, you will provide high-quality patient care and technical skilled care in the patient's home, working closely with our interdisciplinary care team to develop and implement individualized care plans.

Responsibilities:

  • Provide technical care to patients, including administering medications, performing procedures, and providing wound care.
  • Observe, record, and report all observations of the patient's mental and physical condition.
  • Participate in the development and implementation of individualized care plans, working collaboratively with the patient, family, and healthcare team.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Perform special procedures and treatments as instructed and supervised by an RN.
  • Notify the Patient Care Coordinator and/or physician immediately of any changes in the patient's condition.

Requirements:

  • Current LPN licensure in the state of operation.
  • At least one year of nursing experience, with a strong understanding of technical skills and procedures.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively.
