Job Summary:

We are seeking a skilled Social Worker Care Coordinator to join our team. In this role, you will work closely with patients, families, and healthcare providers to optimize the discharge planning process. Your expertise will help ensure a smooth transition from hospital to home, addressing the physical, emotional, and environmental needs of our patients.

Key Responsibilities:
  • Assess patients' and families' psychosocial risk factors to identify areas of need.
  • Develop and implement individualized care plans to address emotional, social, and financial challenges.
  • Collaborate with interdisciplinary teams to ensure timely discharge and post-discharge care.
  • Provide counseling, education, and support interventions to enhance patients' and families' coping abilities.
  • Advocate for patient and family empowerment, independence, and autonomy in healthcare decision-making.
  • Access and mobilize community resources to meet identified needs.
Requirements:
  • LCSW or LSW licensure required.
  • Masters of Social Work from a Council on Social Work Education-approved program.
  • Preferred 1 year hospital social work or related healthcare experience.
