About the Role:

  • As a Nurse Care Manager, you will play a critical role in facilitating the discharge planning process from admission to discharge in collaboration with the healthcare team.
  • You will intervene with patients who have complex or high-risk psychosocial needs as assigned.
  • Perform duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.

Responsibilities:

  • Collaborate with the healthcare team to develop and implement effective transition plans.
  • Conduct thorough assessments of patients' needs and develop individualized care plans.
  • Coordinate with healthcare providers to ensure seamless care transitions.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Provide education and support to patients and their families to promote healthy behaviors and self-care.

Requirements:

  • Graduate of an accredited School of Nursing, required.
  • Current License in the state of employment, required.
  • 2-3 years acute care hospital setting, preferred.
  • Care Management or Utilization Management experience, preferred.
