Key Responsibilities:
- Assessment and Coordination of Care: Conduct thorough assessments of patients, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless transitions of care.
- Patient Education and Support: Provide comprehensive education and support to patients and their families, empowering them to make informed decisions about their care.
- Collaboration and Communication: Foster strong relationships with patients, families, and healthcare professionals through effective communication and collaboration.
- Adherence to Policies and Procedures: Adhere to hospital policies, industry standards, and best practices to promote an optimal patient experience.
Requirements:
- Education: Associate degree in nursing.
- Certification and Licensure: RN-Registered Nurse for the state where care is provided, BLS-Basic Life Support Healthcare Provider, ACLS-Advanced Cardiac Life Support, PALS-Pediatric Advanced Life Support, NIHSS-National Institutes of Health Stroke Scale within 90 days, and TNCC-Trauma Nursing Corp Course, Emergency Department within 180 days of hire if required by entity.
- Experience: 2 years of recent relevant experience.
- Skills and Knowledge: Demonstrated general knowledge and competence with regard to nursing theories, concepts, and practices; medical terminology; anatomy; and physiology. Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
Working Conditions:
- Job Shift: Days.
- Schedule: Part Time.
- Shift Hours: 8.
- Days of the Week: Variable.
- Weekend Requirements: Every other Weekend.
