Our collaborative interdisciplinary team (IDT) approach is fundamental to our values, culture, and mission.
The Geriatric Care Physician conducts thorough in-person evaluations and collaborates with the IDT to formulate personalized care plans for each participant. Under the guidance of the Medical Director, the Geriatric Care Physician supervises the overall care of participants in various settings, including nursing homes and hospitals, managing admissions and discharges for both short-term and long-term placements, and coordinating with medical specialists to ensure seamless care delivery. Additionally, the Geriatric Care Physician provides leadership and support to Nurse Practitioners and other IDT members.
Key Responsibilities:
- Conduct assessments, develop care plans, and provide ongoing medical care to participants.
- Work in conjunction with community physicians and the IDT to facilitate discharge planning from acute or long-term care facilities.
- Actively engage in IDT meetings, sharing updates on participant conditions, collaborating on care strategies, and ensuring 24-hour care coordination.
- Maintain regular visits to nursing home participants based on their needs.
- Participate in a rotating schedule for night and weekend on-call duties.
Qualifications:
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate Degree in Medicine.
- Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics (preferred).
- Active California Physician License without restrictions.
- Reliable transportation.
- Current state CDS Certification and DEA registration.
- Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.

Compensation for this role ranges from $250,000 to $280,000 annually, plus bonuses and equity. Actual compensation will be adjusted based on experience and qualifications.
