Care Manager

Vaya Health is seeking a highly skilled Care Manager to join our team. As a Care Manager, you will be responsible for providing proactive intervention and coordination of care to eligible Vaya Health members and recipients.
Key Responsibilities:- Conduct comprehensive bio-psycho-social assessments to identify member needs and develop person-centered care plans.
- Link members with appropriate and necessary formal and informal services and supports across all health domains.
- Collaborate with members, care teams, and other stakeholders to ensure seamless care coordination and transitions.
- Provide clinical assessment, care planning, and interdisciplinary care team coordination.
- Ensure compliance with HIPAA requirements and maintain confidentiality of member information.
- Participate in routine multidisciplinary huddles and high-risk multidisciplinary complex case staffing as needed.
- Monitor provision of services to informally measure quality of care delivered by providers and identify potential non-compliance with standards.
- Master's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
- Valid licensure required, including Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or other acceptable licenses.
- Two years of experience working directly with individuals with behavioral health conditions, or two years of prior Long-term Services and Supports and/or Home Community-Based Services coordination, care delivery monitoring, and care management experience.
- Ability to drive and sit for extended periods of time, including in rural areas.
- Exceptional interpersonal skills and ability to represent Vaya Health in a professional manner.
- Experience working directly with individuals with intellectual or developmental disabilities or traumatic brain injury.
- Knowledge of the NC Medicaid program, NC Medicaid Transformation, Tailored Plans, state-funded services, and accreditation requirements.
- Ability to complete and maintain all trainings and proficiencies required by Vaya Health.
