RN Coordinator for Hematology and Oncology

We are seeking a highly motivated and experienced Clinical RN Coordinator-Oncology/Hematology to join our team at Nuvance Health Med Practice CT. This role will play a critical part in providing high-quality patient care to our high-risk disease-specific population.
As a Clinical RN Coordinator-Oncology/Hematology, you will be responsible for coordinating clinical and administrative activities, working closely with supervising physicians, infusion staff, and other healthcare professionals to ensure seamless care transitions and effective medication management.
The successful candidate will have a strong background in oncology nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment. Key Responsibilities:
- Develop and implement individualized treatment plans, incorporating evidence-based practices and patient preferences.

- Monitor and report adverse reactions, near misses, and other safety concerns to ensure patient safety and well-being.
- Collaborate with multidisciplinary teams to develop and implement performance improvement initiatives, promoting best practices and quality patient care.
- Stay current with regulatory requirements, accreditation standards, and professional guidelines to ensure compliance and excellence in patient care.
- Lead and participate in educational programs, workshops, and conferences to enhance knowledge and skills in oncology and hematology.
