Job Summary
The Nurse Care Specialist will be responsible for providing high-quality nursing care and support to our participants. This role requires strong communication and problem-solving skills, as well as the ability to work independently and as part of a team.
Key Responsibilities
- Provide nursing care and support to participants, including troubleshooting, advising, teaching, and coordinating care.
- Evaluate participants for episodic illness after business hours in collaboration with the Provider.
- Document care provided and changes in participant's condition in a timely manner.
- Provide nursing care and prescribed treatments.
- Demonstrate nursing care to participants and caregivers/families.
Requirements
- Graduated from an accredited nursing school.
- Minimum of one year of experience working with a frail or elderly population, preferably in long term care, home care, or community health.
- Experience in teaching patients and other healthcare workers is preferred.
- Registered Nurse licensed in the state of Pennsylvania.
- Possess a valid PA Driver's License.
