Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time position that requires strong nursing skills, excellent communication, and a passion for delivering high-quality patient care.

Job Summary:

The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with healthcare providers, and coordinating care with other healthcare professionals. This role requires strong organizational skills, attention to detail, and the ability to work independently with minimal supervision.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' needs
  • Provide education and support to patients and their families
  • Collaborate with healthcare providers to ensure seamless care coordination
  • Document patient progress and outcomes
Requirements:
  • Current RN licensure in the State of Ohio
  • 1-2 years of experience in home health care or medical-surgical nursing
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and CPR certification
