Job Summary

We are seeking a compassionate and skilled RN/LPN to join our team in our state-of-the-art Assisted Living facility at Westminster Village. As an RN/LPN, you will provide direct patient care, administer medication, perform assessments, and work collaboratively with our interdisciplinary team to deliver exceptional care to our residents.

Key Responsibilities
  • Assist the Director of Assisted Living with 24-hour accountability for resident safety and care.
  • Perform direct patient care to residents according to physician's orders and in conformance with recognized nursing procedures, established standards, and administrative policy.
  • Administer medications and treatments as prescribed.
  • Follow the plan of care for each resident within the guidelines of Assisted Living.
  • Work in collaboration with Physicians, Pharmacists, Dietitian, Therapists, and Activities as well as families and other authorized persons and disciplines involved in the care of the resident to facilitate communication and execution of the total plan of care.
  • Observe and encourage infection control measures in accordance with facility policy as well as the rules and regulations governing infection control practices.
  • Observe and promote the facility goal of 'Restraint Free Environment' and assist others in finding alternative measures.
  • Observe and promote the facility goal of 'Limited Lift Environment'.
  • Protect patient confidentiality in all aspects of care and all communication of information, computer communication, telephone communication, and direct communication MUST be handled in strict confidence according to the laws which govern Patient Confidentiality.
  • Perform and document assessments of residents, including behavior monitoring, physical assessment, and response to specific medications, etc.
  • Assist physician in examination of resident or with treatments being performed by the physician.
  • Respond to emergency calls to the Village or Willows if needed.
  • Perform other duties as assigned by the Director of Assisted Living or Memory Care Coordinator.
Requirements
  • Must be a graduate in good standing of an accredited school of nursing.
  • Current and unencumbered license to practice in the State of Illinois as a Registered Professional Nurse.
  • Work experience in Gerontological Nursing is strongly desired. Medical/Surgical experience is acceptable.
  • Working knowledge of healthcare delivery in the Geriatric, Rehabilitation, or Skilled Nursing setting.
  • Knowledge of the aging process, pharmacology, assessment of the elderly, and practical application of clinical interventions.
  • Desire to work in a skilled/rehabilitation setting is required.
  • Current CPR certification is required.
