About the Job:

• Provide one-on-one physical therapy services to patients in their homes, focusing on regaining mobility, strength, and functionality.
• Collaborate with a multidisciplinary team to develop and implement patient care plans, ensuring seamless communication and coordinated care.
• Assess patient needs, document progress, and modify care plans as needed to achieve optimal patient outcomes.
• Educate patients and their families on exercises, self-care, and safe transfer techniques to promote independence and empowerment.
