Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Nurse. As a Home Health Nurse, you will provide nursing visits to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide nursing visits to patients in their homes, assessing their physical and emotional needs and developing and implementing individualized care plans.
  • Work closely with our interdisciplinary team, including physicians, therapists, and social workers, to coordinate patient care and ensure seamless transitions.
  • Administer medications and treatments as prescribed by physicians, and provide education to patients and their families on disease management and self-care.
  • Document patient progress and outcomes in accordance with company policies and procedures.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • One year of clinical experience and one year of home health experience.
  • Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records.
