Job Summary:

As a LPN Home Health Care Specialist with EvergreenHealth, you will provide nursing services to patients in their homes, working under the direct supervision of a registered nurse and following a plan of treatment ordered by a physician.


Key Responsibilities:
Provide direct patient care in the patient's home
Coordinate patient and family/significant other education process
Continuously plan, implement, and evaluate patient care, incorporating patient and family/significant other into the process
Participate with physician partners and other interdisciplinary team members to develop and implement patient and family/significant other plan of care
Communicate effectively all relevant information to immediate supervisor and/or RN case manager, in a timely way

Requirements:
Currently licensed as a LPN in the State of Washington
Obtain an NPI number within 30 days of starting in the position
Current Washington State Drivers license and proof of insurability
Current Healthcare Provider BLS certification by date of hire
Reliable transportation
Demonstrates ability to communicate clinical/educational knowledge to others
Demonstrates skills to complete the nursing competency assessment checklist
Demonstrates knowledge of new trends and techniques in nursing
