About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as an On-Call RN. This role requires a strong commitment to providing high-quality patient care and working collaboratively with our interdisciplinary team.

Responsibilities
  • Provide on-call services after hours, completing skilled visits as needed.
  • Coordinate hospice care by working with other team members and patient family members to ensure needs are met.
  • Triage calls, coordinate services, and provide frequent updates on plan of care and any changes in condition.
  • Assist in reinforcing patient and family education regarding end-of-life care.
  • Participate in IDG and case conferences as needed.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and in-service programs.
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development through further education, workshops, and conferences, as well as active participation in professional and related organizational meetings and individual research reading.
  • Respond to on-call or medical emergencies (nights and weekends).
