About the Role

We are seeking a skilled and compassionate Home Health LPN to join our team at HCA Midwest Healthcare at Home. As a Home Health LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities

  • Administer skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care.
  • Identify the physical, psychosocial, and environmental needs of patients and provide care accordingly.
  • Verify the Plan of Care prior to each visit and provide care based on the physician's orders, assessment data, established standards, and guidelines.
  • Communicate significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
  • Assess patient/caregiver's knowledge of the disease process, treatment regimen, and/or medication administration, and provide basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs.

Requirements

  • Basic Cardiac Life Support certification must be obtained within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Driver's license.
  • Vocational school graduate.
