Job Summary

We are seeking a highly skilled and experienced RN Care Manager to join our team at Marysville Care Center. Key Responsibilities
  • Coordinate patient care and services, including case management, utilization review, and discharge planning.
  • Work collaboratively with patients, families, care teams, payers, and external entities to promote quality care and ensure a safe transition.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.
  • Stay up-to-date on relevant laws, regulations, and industry standards to ensure compliance and best practices.
Requirements
  • Current RN licensure in good standing.
  • One year of clinical experience in post-acute care, preferably in case management or utilization review.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
  • Ability to work independently and as part of a team, with a focus on collaboration and customer service.
  • Proficiency in relevant computer systems and software, including electronic medical records.
Preferred Qualifications
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC).
  • Experience with care coordination, case management, and utilization review.
  • Knowledge of relevant laws, regulations, and industry standards.
