About the Role

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Home Risk Assessment Specialist, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes.

This is a unique opportunity to leverage your expertise and make a meaningful impact on the health and well-being of others. You will work collaboratively with physicians, case managers, and other healthcare professionals to provide high-quality care and improve patient outcomes.

Key Responsibilities:

  • Conduct thorough health assessments, including medical history, diagnosis, and treatment planning
  • Work with patients, families, and caregivers to develop personalized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families on health management and disease prevention

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner
  • Strong communication and interpersonal skills
