About the Role

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at Revival Home Health & Hospice. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Assess and analyze patient care needs to develop comprehensive plans of care, including discharge planning.
  • Conduct thorough assessments of the patient's environment to ensure a safe and effective delivery of care.
  • Identify therapy diagnosis/patient care problems and interventions, making additions or changes as necessary to ensure comprehensive addressing and progress documentation.
  • Implement and interpret physician orders, ensuring professional ethics, patient rights, and state and federal codes and regulations are met.
  • Initiate and coordinate the plan of care, integrating care with other disciplines as needed.
  • Document additions and/or changes to the patient plan of care, re-evaluating progress and making necessary changes under physician direction.
  • Provide patient and caregiver education on diagnosis, medications, infection control, and safety measures in accordance with the plan of care.
  • Conduct comprehensive medication reviews, ensuring patients and caregivers receive information on medication regimens, including frequency, duration, use, side effects, indications, and contraindications.
  • Communicate with other disciplines to ensure collaboration, coordination of care, and enhanced patient outcomes.
  • Provide all aspects of case management to enhance patient outcomes.
  • Complete and submit required documentation within established agency guidelines, ensuring compliance with state/local/payer requirements.
  • Requisition and manage equipment and supplies required to increase client function and independence.
  • Demonstrate teaching skills and continued competence in performing therapeutic interventions for all levels of complexity in patient care.
  • Supervise occupational therapy assistants and home health aides.
  • Coordinate all aspects of patient care with internal and external care providers.
  • Instruct patients/caregivers on safety measures appropriate to care.
  • Maintain universal precautions and infection control standards to prevent the transmission of infections and communicable diseases.
  • Recognize changes in patient status and report accordingly.
  • Provide clear, concise, and complete reporting of patient status in compliance with agency and regulatory standards.
  • Complete documentation in compliance with agency and regulatory standards.
  • Demonstrate proficiency in OASIS documentation, including timeframes and response accuracy.
  • Report all incidents per agency and regulatory standards.
  • Follow policies and procedures according to agency, state, and regulatory standards.
  • Use safe body mechanics to prevent personal injury to self or patient.
  • Function within the Scope of Practice.
  • Attend and participate in staff meetings, patient care conferences, in-services, and other meetings as requested.
  • Maintain confidentiality of patient-related information and honor patient rights.
  • Participate in the agency's QAPI program by communicating problem areas identified and recommendations to address problem areas; data collection; attendance at periodic QAPI meetings; and participation in performance improvement projects as requested.
Requirements
  • Graduate of an Occupational Therapist Education program approved by ACOTE.
  • Current Occupational Therapist licensure in the State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire.
  • Two (2) years of experience in adult occupational therapy, home health experience preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and occupational therapy skills per competency checklist.
