We are seeking a compassionate and skilled Home Health Aide to join our team at Trinity Health. As a Home Health Aide, you will provide personal care assistance to seniors in their homes, maintaining their independence and dignity.
Responsibilities:
Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming.
Assist with physical activity, exercise, and walking.
Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the center.
Responsible for participant safety, identifying safety hazards and emergency situations, and initiating corrective action as able.
Provide behavioral supervision or support for participants, including reality orientation, reminders to follow through with nursing and medication instructions, diet restrictions, and emotional reassurance.
Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor.
Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participants' expressed wishes.
Prepare and submit timely written reports as required.
Attend staff meetings and participant care planning on a regular basis.
Requirements:
High school diploma or GED required.
Active certified nurse assistant in the state where PACE Organization is located preferred.
Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting. Must be able to push, pull, move, and/or lift a minimum of 20 pounds if applicable.
Physical mobility that includes movement from place to place.
Must be able to sit for long periods of time.
Physical agility, including the ability to maneuver the body while in place.
Dexterity of hands and fingers.
Coordination, including eye-hand, hand-foot.
