Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Sunrise Senior Living in Golden Valley, Minnesota. As a LPN, you will play a vital role in providing high-quality care to our residents, promoting their overall well-being, and enhancing their quality of life.

Responsibilities:
  • Conduct regular wellness visits for all residents, monitoring their health and providing necessary care and support.
  • Assist in maintaining accurate and up-to-date resident wellness files, ensuring compliance with federal and state regulations.
  • Collaborate with the Resident Care Director (RCD) to conduct resident evaluations and gather data for quality improvement initiatives.
  • Provide clinical support and assistance to community team members as needed, promoting a culture of teamwork and collaboration.
  • Participate in Sunrise Quality Assurance Meetings and Clinical Meetings, contributing to the development and implementation of quality care initiatives.
Qualifications:
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations.
  • Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
