Job Summary

We are seeking a skilled Certified Nursing Assistant to join our dynamic care team at Plymouth Rehabilitation & Health Care Center. As a key member of our team, you will play a vital role in providing exceptional patient care and support to our residents.

Key Responsibilities
  • Provide Routine Daily Nursing Care: Assist residents with daily living activities, such as bathing, dressing, and grooming, in accordance with established nursing care procedures.
  • Participate in Care Planning: Collaborate with licensed staff nurses to develop and implement individualized care plans that meet the unique needs of each resident.
  • Document Resident Care: Accurately and thoroughly document resident care activities, including vital signs, medication administration, and any changes in resident condition.
  • Meet Resident Needs: Respond promptly to resident call lights and ensure that all residents have access to necessary care and support.
  • Contribute to a Positive Work Environment: Demonstrate a positive and respectful attitude, and work collaboratively with other team members to maintain a safe and supportive work environment.
Requirements
  • Certification and Education: Must be a registered certified nurse's aide in accordance with state laws, and have a high school diploma or equivalent.
  • Experience and Skills: General work experience in long-term care is preferred, but not necessary. Must possess excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
  • Physical and Mental Demands: Must be able to lift, push, and pull residents and equipment, and be able to work in a physically demanding environment.
