About Our Organization

DFD Russell Medical Centers operates four community health centers in central Maine, serving a multicultural, rural population of approximately 10,000 patients. Our health centers have a family practice focus, serving patients from pediatrics to geriatrics.

Job Summary

The Care Manager will provide high-quality patient care, coordinating with primary care providers, specialty practices, and external support services to ensure seamless transitions and optimal health outcomes.

Key Responsibilities
  • Provide depression screening and health status assessments
  • Reconcile medications and assist with transitions of care
  • Perform annual wellness visits, weight management counseling, and diabetes education
  • Develop and implement advance care plans
Requirements
  • Current RN license with the ability to practice in Maine
  • Previous Care Management experience preferred
  • Knowledge of EMR systems, with Centricity or Athena One preferred
  • BLS certification required, ACLS certification preferred
Benefits
  • Competitive hourly rate of $32.00
  • 40 hours per week
