Job Duties:
Conduct nursing assessments for all patients within 8 hours of admission. The information gathered forms the foundation of a personalized care plan. Identify educational needs and objectives related to each patient's diagnosis through collaboration with the patient/family and the interdisciplinary team.
Provide timely patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences. Responsible for accurate, concise, and timely documentation of all assessments, interventions, and patient responses in the medical record, adhering to policy.
Coordinate patient care in partnership with a diverse range of healthcare professionals.
Facilitate optimal outcomes concerning clinical care, quality, and cost-effectiveness.
Ensure compliance with established standards of care and practice in accordance with all policies, procedures, and guidelines utilized in the medical treatment facility.
Perform physical examinations and health histories.
Deliver health promotions, counseling, and education.
Administer medications, wound care, and various other personalized interventions.
Direct and supervise care provided by other healthcare professionals.
Accountable for assigning patient care based on the scope of practice and skill level of assigned personnel.
Recognize adverse signs and symptoms and respond swiftly in emergency situations.
Communicate and collaborate with a diverse group of individuals to inform the healthcare team of plans/actions and to educate the patient/family and organization.
Schedule referral appointments and arrange specialty care as necessary.
Perform nursing services as identified in the task order.
Engage in research to support improved practices and patient outcomes.
Prioritize activities and plan for effective use of time and staff to meet patient needs.
Perform additional duties as requested.
