This is a full-time exempt position that requires the ability to work independently and as part of a team. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.

Responsibilities

  • Nursing Care">
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and pain control.">
  • Assess patient needs and develop individualized care plans to meet those needs.">
  • Collaborate with other healthcare professionals to ensure seamless transitions of care.">
  • Admissions and Initial Assessments">
  • Conduct initial assessments of patients to determine their level of care needs.">
  • Develop and implement individualized care plans based on patient assessment data.">
  • Communication and Documentation">
  • Communicate effectively with patients, families, and healthcare professionals to ensure timely and accurate transfer of information.">
  • Document all interactions with patients and families in the electronic medical record.">
  • Teamwork and Collaboration">
  • Work collaboratively with other members of the interdisciplinary team to achieve patient-centered goals.">
  • Participate in ongoing education and training to stay current on best practices in hospice care.">

Requirements

">
  • Graduate of an accredited school of nursing.">
  • Current licensure to practice as a Registered Nurse in the State of Texas.">
  • Minimum one year of experience in hospice or palliative care.">
  • Strong communication and interpersonal skills.">
  • Ability to work independently and as part of a team.">
  • Ability to navigate various computer systems and software.">
  • Knowledge of state and federal regulations governing hospice care.">

Work Environment

">

Must be able to perform under a wide variety of conditions and circumstances in the home environment. Must possess sufficient manual dexterity to handle IV procedures, specimens, equipment, and medications. May be required to repeatedly lift, carry, push, and pull greater than 50 pounds and carry agency-provided electronic equipment for completing the Electronic Medical Record (EMR) in the home.
