Job Summary

We are seeking a skilled and experienced Licensed Practical Nurse (LPN) to join our team in providing pediatric home health care services. As an LPN with Aveanna Healthcare, you will have the opportunity to work with a diverse range of patients and families, and to make a meaningful difference in their lives.

Responsibilities
  • Deliver high-quality patient care in the home setting, in accordance with physician orders and Aveanna Healthcare's policies and procedures.
  • Conduct thorough assessments of patient needs and develop individualized care plans to meet those needs.
  • Provide education and support to patients and their families, to promote health and wellness and to empower them to take an active role in their care.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and to achieve optimal patient outcomes.
  • Document all patient interactions and care activities in a timely and accurate manner, to ensure compliance with regulatory requirements and to facilitate quality improvement initiatives.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have a strong sense of smell and touch.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Environment
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
