Job Summary

Alternate Solutions Health Network LLC is seeking a skilled Registered Nurse to join our team as a Home Health Case Manager. This role involves providing high-quality patient care in a home setting, working closely with patients, families, and healthcare teams to ensure optimal outcomes.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Provide skilled nursing care in a patient's home, as directed by the attending physician
  • Collaborate with interdisciplinary teams to ensure continuity of care and promote positive patient outcomes
  • Administer medication as prescribed and help minimize patient risk through prioritized visits for high-risk patients
Requirements
  • Current RN license in the state of employment
  • Minimum one year of experience as an RN in an acute care setting, with home care experience preferred
  • Valid driver's license and auto insurance
  • Ability to meet the physical demands of the job
