Pediatric Primary Care Provider

POSITION OVERVIEW:

The Pediatric Primary Care Provider delivers comprehensive medical services to patients, adhering to established medical standards and organizational protocols.

KEY RESPONSIBILITIES:
  • Delivers direct healthcare services focused on pediatric patients.
  • Evaluates, plans, documents, and reviews patient care needs and interventions, ensuring appropriate follow-up.
  • Maintains a culturally and linguistically responsive practice, utilizing interpreters and other resources as necessary.
  • Creates individualized care plans based on thorough assessments of physical, emotional, and social factors, collaborating with patients and their families when appropriate.
  • Refers patients to specialists and other healthcare providers as needed.
  • Reviews patient records to ensure the development and adherence to effective treatment plans.
  • Exhibits sensitivity and responsiveness to both verbal and non-verbal cues from patients, families, and colleagues.
  • Upholds patient confidentiality and dignity in all aspects of care delivery, maintaining discretion in communications.
  • Utilizes sound judgment in addressing clinical and non-clinical challenges.
  • Collaborates with other healthcare professionals in patient education and rehabilitation, promoting overall health and wellness.
  • Fosters cooperative relationships with fellow physicians, administrative staff, and clinical teams, providing guidance and support as needed.
  • Conducts patient visits in hospital settings as required and participates in on-call rotations.
  • Engages with the Patient Care Committee to enhance the quality of care provided.
  • Full-time providers are expected to manage a minimum of 4200 patient visits annually.
HIV CARE:
  • Offers ongoing education and support to patients living with HIV regarding their treatment plans, including medication adherence and side effects.
  • Educates families and support networks about HIV/AIDS and treatment options as identified by the patient.
  • Analyzes laboratory results and provides clinically appropriate treatment recommendations.
  • Participates in a multidisciplinary healthcare team.
  • Assists patients and families in navigating challenges related to illness and HIV diagnosis.
  • Collaborates with the HIV Team and Consumer Advisory Board to ensure culturally appropriate services.
  • Documents all patient interactions and care activities.
QUALITY IMPROVEMENT:
  • Engages in continuous evaluation and enhancement of care practices.
  • Contributes to the development, review, and revision of organizational policies and procedures.
PROFESSIONAL DEVELOPMENT:
  • Actively participates in scheduled staff meetings.
  • Engages in both formal and informal teaching opportunities for staff.
  • Maintains clinical expertise through ongoing education and training.
  • Retains admitting privileges at relevant medical facilities.
QUALIFICATIONS:
  • Possesses current medical licensure and relevant certifications.
  • Board certified or eligible in pediatrics, with a commitment to pursuing board certification if not yet achieved.
  • Ability to collaborate effectively within a multidisciplinary team, including mid-level providers, social workers, and case managers.
  • Fluency in Spanish, Portuguese, French, or Haitian Creole is highly desirable; prior experience in ambulatory care or public health is preferred.
  • Proficient in electronic medical record systems.
WORK ENVIRONMENT:

A dynamic community health center situated in a vibrant urban area.

May require occasional extended hours.

Potential exposure to patient bodily fluids.

Some travel may be necessary for professional conferences.

PHYSICAL DEMANDS:

Physical tasks may include lifting light equipment and boxes (up to 20 pounds). Primarily sedentary with some interoffice mobility required. Visual and auditory acuity sufficient for reading and conversation is necessary.

TRAVEL AND WORK HOURS:
  • Occasional travel for continuing medical education or conferences may be required.
  • Includes potential evening and weekend hours.
  • On-call responsibilities during designated periods.
