Job Summary: We are seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing patient-centered care, working closely with our interdisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities:
- Deliver high-quality patient care: Apply your knowledge of nursing theories and concepts to provide individualized care to patients and their families.
- Coordinate patient care: Work with the patient, family, physician, and Hospice team to ensure seamless care coordination.
- Communicate effectively: Communicate with patients, families, and healthcare team members at all levels to ensure clear understanding and expectations.
- Participate in IDT meetings: Attend and participate in Interdisciplinary Team meetings to discuss patient care plans and ensure quality care.
- Meet reimbursement guidelines: Ensure that patient care and documentation meet reimbursement guidelines.
Requirements:
- Current RN licensure: Possess current RN licensure in the state of practice.
- Nursing experience: Have at least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
- Knowledge and skills: Possess knowledge of the framework, function, and organization of a home health and hospice care program, as well as the nurse's role as a member of the patient care team.
- Education and certifications: Hold a nursing education from an approved school of nursing and maintain CPR certification.
- Travel and flexibility: Be willing to accept travel assignments and work flexible hours, including weekends, evenings, and holidays.
