About the Role

Key Responsibilities
  • Integrate healthcare services for clients from pre-admission to post-discharge, coordinating and sequencing care to enhance patient flow and interdisciplinary communication.
  • Evaluate outcomes based on integration among clinical, financial, and utilization data, utilizing independent nursing judgment to inform decision-making.
  • Work in structured and unstructured healthcare settings, applying knowledge of age-specific care methodologies and protocols to meet the diverse needs of clients.
  • Communicate effectively with patients, families, and healthcare teams, coordinating and facilitating care to achieve optimal outcomes.
  • Monitor, evaluate, and trend patient responses, controlling variances and duplication to ensure fiscally responsible care.
Requirements
  • Bachelor's degree in nursing preferred, with a minimum of one to two years of professional nursing experience in a clinic care setting.
  • Graduate from a nationally accredited nursing program, with in-depth knowledge of specialty area preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing, with the ability to obtain and maintain required department-specific competencies and certifications.
